Prime Healthcare Named One of the Nation’s
Top 10 Health Systems for Patient Safety

Recognition underscores Prime’s physician-led commitment to safety, quality, and compassionate care across one of the nation’s largest community health systems.

 

(Ontario, Calif., November 18, 2025) – Prime Healthcare announced today that 24 of its hospitals across 11 states earned an “A” Hospital Safety Grade from The Leapfrog Group for Fall 2025, reaffirming the system’s position among the nation’s top-performing health systems for patient safety, clinical excellence, and quality outcomes.

According to The Leapfrog Group’s Fall 2025 report, Prime Healthcare is one of only ten health systems nationwide with the most hospitals earning an “A” Hospital Safety Grade and ranks among the top five for Straight A performance. Overall, 53% of Prime hospitals received an “A,” a higher percentage than nearly every other health system in the country.

As one of the largest physician-founded and physician-led health systems in the United States, the Prime Healthcare continues to set the standard for transforming community hospitals into centers of excellence that provide accessible, high-quality, compassionate care. The system’s most recent recognitions demonstrate its ongoing investment in safety, innovation, and quality.

Among the 24 Prime hospitals receiving an “A” grade, 20 have earned consecutive top marks across multiple grading periods, demonstrating a sustained culture of safety and quality. East Liverpool City Hospital in Ohio has achieved straight “A” ratings since 2018, while Providence Medical Center in Missouri has maintained its top-tier performance since 2019.

The Leapfrog Hospital Safety Grade is the nation’s only ratings program focused exclusively on preventable medical errors, infections, and injuries. Grades are peer-reviewed, fully transparent, and updated twice annually, in the spring and fall.

In 2025, Prime hospitals earned more than 150 clinical quality awards from Healthgrades and have been named among the nation’s “100 Top Hospitals” by Premier a total of 72 times.
