Prime Healthcare is pleased to participate in a Medicare Accountable Care Organization (ACO) model offering high quality care to our patients with Medicare. ACOs are groups of doctors, hospitals, and other healthcare providers who collaborate to provide coordinated, high-quality care to their Medicare patients. REACH stands for Realizing Equity, Access, and Community Health.
Through ACO REACH, people with Medicare will receive more access to services through better care coordination, reaching and connecting healthcare providers and beneficiaries, including those who are underserved.
The model is focused on improving overall care in a safe environment, effective care coordination, and lowering costs.
